Determine Eligibility
To access case management services, individuals must have an intellectual or developmental disability as defined by the Missouri Department of Mental Health Division of Developmental Disabilities. Eligibility is determined based on the following criteria:
- The presence of a mental and/or physical disability that originated before the age of 22.
- A condition likely to continue indefinitely, resulting in functional limitations in two or more life activities.

“Persons with developmental disabilities” means individuals who have a severe, chronic disability which is attributable to:
- Intellectual disability, cerebral palsy, epilepsy, head injury or autism, or a learning disability related to a brain dysfunction
- Any other mental or physical impairment or combination of mental or physical impairments
- Is manifested before the person attains age 22
- Is likely to continue indefinitely
- Results in substantial functional limitations in two or more of the following areas of major life activities as determined through Missouri Adaptive Abilities Scale (MAAS):
- Self-care
- Receptive and expressive language development and use
- Learning
- Mobility
- Self-direction
- Capacity for independent living or economic self-sufficiency
